As a valued supporter we really want to keep you posted about news and events across St Gemma’s and to share how your support benefits local patients and their families.  
 
We will contact you for administrative purposes regarding this interaction. Declaration
DECLARATION:
By registering I agree to make the Hospice aware of any dietary requirements of any guests and understand that if I need to make any changes to my requirements I can use the contact details in the confirmation email I will receive.  

I am aware that tickets will not be posted out and that once booked St Gemma’s will have my details and will have allocated my table and seat.

I agree that I will behave appropriately at all times when travelling to, from and during the event. I understand that my behaviour at the event may reflect upon the Hospice and I acknowledge that anyone behaving inappropriately may be asked to leave the venue at the sole discretion of St Gemma’s Hospice.

CANCELLATION
I understand that my ticket(s) will be fully refundable if I am unable to attend up until two weeks before the Ball. If I need to cancel my ticket(s) within two weeks of the event date then I understand that refunds will not be available.  

YOUR DATA:
I understand that photographs may be taken at the event and I consent that any photographs taken that include me may be used for future Hospice marketing purposes and in publications.
